"AHardwarerejuvenation"- today this sentence is on everyone’s lips. Use hardware influences indermatologyAlsoremoveSkin-ManifestationsseriousDiseases. We have an army of beauty at our service! Does it serve us faithfully? "CONCENTRATION CAMP"talkedWithexpertprofessorIgor Pinson.
During the interview, Igor Yakovlevich Pinson shows slides on a computer monitor, leafs through books on medicine and tours the practices: "We have all the hardware methods used in dermatology. Excimer laser, excimer light, all kinds of phototherapy, various kinds of blue light, all kinds of laser and non-laser energy sources, radio wave sources. ". . . Our interlocutor is a treasure of scientific knowledge (his doctoral thesis is dedicated to correcting the immunopathological mechanisms of psoriasis using an excimer laser), holder of rich practical experience and a professional who is infinitely passionate about his work: "Just like itThere is no second Taganka Theater or Lenkom, so a cosmetic clinic can be one of many. " Art! "
In addition to running a high-tech clinic, Igor Yakovlevich is involved in training doctors and is editor-in-chief of the renowned medical journal "Laser Dermatology and Cosmetology" (our version of a foreign magazine). Together with several companies, it participates in research andDevelopment of new methods: "We recently tested the method of using cold plasma as an alternative to laser exposure. "
The clinic is completely specialized in dermatology. Surely you also take on difficult cases?
I. P. :We take on the most difficult and complex cases. This is not just psoriasis, vitiligo, acne, rosacea. We treat patients with Devergie's disease (pityriasis versicolor pilaris) and pemphigus, which many clinics are not allowed to touch at all.
If necessary, we always carry out a thorough examination of the skin using ultrasound and 3D diagnostic procedures. We inform each patient in detail about their illness, take a detailed anamnesis with a description of the local status and prepare a comprehensive report.
Unfortunately, people do not always receive appropriate treatment for their illness. For example, for psoriasis, you may be prescribed fourth-class hormonal drugs - very strong, without having previously taken third-class drugs. . .
Do you have more patients with dermatological diseases? How about solving purely aesthetic problems - eliminating wrinkles, evening out skin tone, rejuvenating?
I. P. :Regarding the ratio of the number of patients, I would say 50: 50. As for cosmetics, the fact is that we often consider it only as a way to rejuvenate ourselves, and do not perceive it as a complex of serious, well-thought-out effects –hence lots of problems! It is no coincidence that another of our specializations is the correction of complications after hardware procedures. And there are many complications – burns, scars, depigmentation, hypopigmentation, hyperpigmentation…
For example, a patient came to us after the simplest procedure - hair removal. With an incredible burning sensation at the tattoo site. The only thing the treating doctor didn't know was that this type of laser treatment cannot "touch" the artificial pigment.
I would like the professional level of some doctors not to be limited to the information they receive from cosmetic equipment sales managers who pursue commercial goals.
Shouldn't a professional have a certificate of professional use of a particular device? Traders say exactly the opposite. . .
I. P. :When you buy a new car, do you take driving lessons again to drive a car of that particular brand? . . . Of course not! You already know what a car is, you are an experienced user.
The same applies to cosmetics: there is the term CO2-Lasers, erbium lasers – these lasers are the same everywhere. The doctor must know in advance what they are.
Of course, each device has its own special features that you need to find out about when purchasing. But a special certificate for each device. . . Only a doctor with specialist knowledge can decide which device manufacturers are acceptable to him. If you lack medical competence, no certificate will help.
If it were up to me, I would greatly reduce the number of institutions that train doctors. I would do it like in the Soviet Union, when there were only a limited number of educational units - with an academic approach, qualified personnel and an adequate material and technical base. Nowadays only the lazy don't teach cosmetology.
In our country, almost all of the literature on hardware methods is published by companies that distribute devices. For the most part, it does not contain educational information, but rather a presentation of the capabilities of a particular device. That's why we have to look at books critically.
I would like to believe that doctors still have the opportunity to improve qualitatively and, so to speak, objectively. Otherwise it will be restless. . .
I. P. :We are currently at the First Moscow State Medical University named after her. Sechenov organized a training center, within which we provide dermatologists and cosmetologists with the knowledge they so urgently need, without any connection to manufacturers of hardware devices (the training center should serve exclusively educational purposes and not pursue marketing interests! ).
Recently we had a great event: we invited Professor Aryeh Orenstein, Director of the Institute for New Medical Technologies and Chairman of the Israeli Society of Plastic Aesthetic Surgeons (read the interview with Dr. Orenstein in the September issue of K&Z. - Ed. ) - This is an undisputed professional authority, the person who was the first in the world to use lasers for cosmetic purposes. The whole day was dedicated to one topic (and not just one device! ) - the correction of vascular formations. The professor gave master classes.
I think it is still important to emphasize that, despite the existing difficulties, we have many excellent dermatologists and cosmetologists.
I. P. :Undoubtedly! Don't exaggerate too much. We really have enough competent and experienced specialists. There are problems, we have pointed them out, but they are not unique.
Incidentally, the situation is no better in the West. For example, if in our country only a doctor, a specialist with higher medical education has the right to carry out laser hair removal, then there it can be carried out by a person who has attended short-term training courses.
Now let's talk about the importance of training doctors. What do you think about the fact that it is also written in specialist publications today: in the foreseeable future, devices will reach such a level of perfection that interventions can be carried out practically without the involvement of a specialist. . .
I. P. :This is fundamentally wrong. Many doctors place completely unjustified hopes on protocols (instructions, prescriptions based on a statistical summary of the manufacturer's medical experience, a large number of clinical cases. - Ed. ) for carrying out procedures.
It's like a surgeon asking for a protocol for a scalpel (although it all depends on his hands, his knowledge of anatomy, etc. )! So there is a physics of the process of interaction between a laser beam and tissue - biophysics that a beautician must know.
Depending on his existing knowledge, he will use one or another hardware method or a combination thereof.
Hardware cosmetology is such an extensive level of knowledge. Maybe it makes sense to have beauticians with a narrow specialization?
I. P. :I do not think so. There is a separate specialty - "cosmetology", and a beautician must be able to do everything that falls within his competence. He cannot be a specialist in the left big toe!
The doctor must have clinical thinking and understand how different methods work together. For example, to correct involutive changes (aging - editor's note) in the skin, some devices are not sufficient; other procedures are also required: biorevitalization, contour plastic. . .
I will continue the topic of medical competence. Recently, one of the prominent specialists on the pages of our publication said that the focus is on a doctor's hands and head. And the novelty of the equipment used can be put in second place. Do you agree with that?
I. P. :No, I don't agree. A doctor's hands and head are necessary, but they are not enough. A smarter doctor using an old device cannot remove a tattoo efficiently. Our direction is evolving very quickly. The technical specifications of the devices change every two years.
Of course, a bad specialist will not do the patient any good, even if he has a sophisticated device. However, there can be no compromise between a highly qualified doctor and not the most relevant equipment.
Everything is in the hands of new technologies! A doctor who keeps a keen eye on innovation will rise to the challenge.
Here in our clinic you will see two versions of the same device with a difference of only one year. Why? Because we value our customers very much and, with all our will and talent, we cannot squeeze a function out of the previous version device that it simply does not have.
Igor Yakovlevich and the last question. Please give recommendations on choosing a clinic to our readers who are thinking about hardware procedures.
I. P. :Of course I say: "Go to a well-equipped specialist clinic and not to a small cosmetic practice with two machines where a little bit of everything is done and hardware procedures are carried out in between. "
A common cause of complications lies in the inadequate equipment of a medical facility: when doctors try to solve certain problems with a device that is not intended for this purpose.
Get as much information as possible about the specialists working in the clinic - thanks to the Internet, this is easy.
Pay attention to who runs the institution: the wife of a wealthy person who wants to become the owner of a beautiful company or a highly qualified person.
Nowadays, the patient must make his choice very responsibly and critically in order to avoid disappointment and trouble!
In a note
- Laser is an acronym: Light amplification by stimulated radiation emission – Light amplification by induced (stimulated) radiation. The history of the laser dates back to 1916, when Albert Einstein developed his theory about the interaction of radiation with matter.
- There are different types of laser treatments. "So we can talk about cold, warm, hot ablation (ablation is the evaporation of the surface layer of the skin with laser radiation. - Editor's note) or no ablation at all, " says I. Ya. Pinson. To eliminate fine wrinkles isone laser procedure is required, to smooth deep skin folds another. The decision about this or that effect is made after a thorough diagnosis.
- Is it better to leave keloid scars (bulging scars) untouched? Many clinics will answer this question with yes. However, keloids (for example a scar caused by an unsuccessful piercing) are removed - and successfully! As always, everything depends on the competence and experience of the doctor, as well as the equipment used.
- Can laser rejuvenation deplete skin resources and lead to premature aging?"This is complete nonsense! "- says I. Y. Pinson. – Unless you are constantly undergoing laser treatment with or without reason. In addition, "drain" is not a medical term. Laser procedures (like all others) must be used strictly according to indications and have a pathogenetic effect. "
